Recognizing Rising Risks for Readmission: Awareness of Sepsis in the Community

Tools and information that can be utilized in the primary care setting will be handed out and made available on the CMP website.

This training will provide you with an overview of the 2017 PCMH and CPC+ behavioral health requirements that were presented at the June 20th QI Round Table meeting.


Care Management Community Resources WebEx

This training will help you prepare for the 2017 PCMH required addition of Social Functioning and Social Determinates of Health to the Patient Comprehensive Health Assessment. The CMP Care Management Advisors and Social Work team will be working closely with you to ensure your team is aware of available resources for population screening and Care Management.
