Patient Centered Medical Home
The Patient Centered Medical Home (PCMH) program recognizes practices that use systematic processes and information technology to enhance the quality of patient care. The program’s standards emphasize the use of systematic, patient-centered, coordinated care that supports access, communication, and patient involvement, while restraining the cost of care. Catholic Medical Partners has been working with primary care practices consistently for many years to achieve PCMH recognition through the NCQA. All Catholic Medical Partners primary care practices are working toward or have already achieved level 3 PCMH recognition.
Increasing the adoption rate of PCMH in our primary care practices enables us to improve our accountability for quality, cost and patient satisfaction. Primary care is foundational to our future success, as is promoting prevention and better care for patients with a higher burden of illness.
